A folio is your account number with a fund house, under which all your holdings in that fund house's schemes are recorded.
When you first invest with a fund house, it opens a folio for you and gives it a number. Later purchases in any of that fund house's schemes can go into the same folio, so one folio can hold several funds.
Each folio records:

Your folio numbers appear on every account statement and on your CAS. Folio numbers explains where to find them and how to update the details in each.
